In many low- and middle-income countries, PEPFAR and the Global Fund are significant funders of commodities to diagnose, treat, and prevent HIV, along with country governments. But, information on their relative contributions, which is important for assessing the HIV response, is not readily available. We analyzed data from PEPFAR documents on funding for key HIV commodities in 34 PEPFAR countries to better understand the funding landscape.
The House Committee on Appropriations released its FY 2022 State, Foreign Operations, and Related Programs (SFOPs) appropriations bill on June 28, 2021 and accompanying report on June 30, 2021. The SFOPs bill includes funding for U.S. global health programs at the State Department and the U.S. Agency for International Development (USAID).
President Biden released his full FY 2022 budget request to Congress on May 28, 2021 (an initial, topline discretionary FY 2022 budget proposal was released on April 9, 2021). The request includes an increase of almost $1 billion for global health programs, most of which (84%) is for global health security.
To provide context for the release of the administration’s first, full budget request for FY 2022, this brief provides an overview of historical trends in U.S. global health funding, including changes in program-specific funding over time, the distribution between bilateral and multilateral support, and in the increasing use of emergency supplemental funding in response to outbreaks.

In this policy watch, we sought to put the U.S. pledge of 80 million doses in further context.
This analysis examines overall COVID-19 vaccination levels and how the rate of vaccinations over the last week has changed at state-level and national level. There is significant variability by state in how many adults in the US have been vaccinated with at least one dose. Most states are also seeing a decline in the pace of vaccinations, indicating that they may be approaching the point when vaccine supply exceeds demand.
As India and other countries continue to grapple with major COVID-19 outbreaks even as cases decline in this country, there is increasing attention to the global role that could be played by the U.S. government.
This brief examines U.S. policy options for helping expand global access to COVID-19 vaccines, focusing on four main areas: in-kind donations of vaccine doses, additional funding for global access mechanisms like COVAX, helping expand global vaccine manufacturing, and relaxing or waiving intellectual property protections on vaccine technologies. We summarize what the administration has done to date in these areas and policy issues related to each.
With supply having increased significantly and eligibility fully open to adults in all states as of April 19, this analysis estimates that COVID-19 vaccine supply will exceed demand across the U.S. over the next 2 to 4 weeks.
