Under estate recovery, state Medicaid programs are required to recover the costs of long-term care and related hospital and prescription drug services for enrollees ages 55 and older. KFF examines the wide variation in estate recovery practices across states as well as the criticisms of this policy, which have led to federal proposals to modify or reduce it.

In April 2023, the Centers for Medicare and Medicaid Services (CMS) released guidance encouraging states to apply for a new Section 1115 demonstration opportunity to test transition-related strategies to support community reentry for people who are incarcerated. As of August 19, 2024, CMS has approved Section 1115 reentry waiver requests from 11 states, while 13 additional reentry waivers remain pending.
On July 16, 2022, the federally mandated crisis number, 988, became available to all landline and cell phone users at no charge. Since launch in July 2022, 988 has received 10.8 million calls, texts, and chats. In May 2024, monthly contacts exceeded half a million, up about one-third from a year ago and 80% since May 2022. Despite increased demand for 988 services, national answer rates improved and wait times decreased, though some gains slipped in the second year.

Using health insurer financial data for 2023, we find that in 2023, per enrollee gross margins were highest in the Medicare Advantage market, and medical loss ratios were lowest in the individual insurance market. Across most markets, gross margins have been relatively stable in recent years.
