The presence of ICE at hospitals and health care facilities represents a reversal in policy under the Trump administration from previous policy that had protected against enforcement in these and other “sensitive locations” like schools and places of worship.
In this JAMA Viewpoints column, KFF's Drew Altman, Ashley Kirzinger and Mollyann Brodie explore the power of health care affordability as an economic issue, how it has played out in recent election cycles, and the implications for the 2026 midterm elections.
Proposals from some Republicans in Congress would effectively repeal some or all of the ACA premium tax credits and replace them with contributions to Health Savings Accounts (HSAs) or something similar. Senator Rick Scott proposes “Trump Health Freedom Accounts” and Senator Bill Cassidy proposes HSA contributions available only for people who enroll in bronze level ACA plans to pay for out-of-pocket health care costs.
Most immigrant voters disapprove of President Trump’s handling of immigration, including large majorities who disapprove of policies like “third country” deportations and efforts to end birthright citizenship. These views may have implications for future elections, as six in ten immigrant voters say the administration’s immigration enforcement actions have had an impact on which political party they support.
Amid the Trump administration’s increased immigration enforcement efforts, one in five immigrants say they personally know someone who has been arrested, detained or deported since January. Four in ten worry they or a family member could face such action. Many immigrants, including naturalized citizens and those who are lawfully present, say they feel less safe, are avoiding activities outside their home, and no longer view the U.S. as a good destination for immigrants.
A new KFF/New York Times Survey of Immigrants reveals deepening anxiety and fear among immigrants of all statuses amid the Trump administration’s intensified immigration enforcement and restrictive policies. Findings from the new survey are detailed in three KFF reports and help inform the reporting in a package of news stories by Times reporters.

Three quarters of the public favor of extending them, though support among Republicans has dipped since September. Most Democrats want a Congressional budget deal to include an extension of the tax credits.
KFF's Health Tracking Poll finds that more than three-quarters of the public say they want Congress to extend the ACA enhanced premium tax credits that are set to expire at the end of this year. If the credits do expire and the average premium more than doubles, seven in 10 of those who purchase their own insurance say they would not be able to afford premiums without significantly disrupting their household finances.
A new KFF Health Tracking Poll finds more than three-quarters (78%) of the public say they want Congress to extend the enhanced tax credits available to people with low and moderate incomes to make the health coverage purchased through the Affordable Care Act’s Marketplace more affordable.
