Indicator based on methodology developed by the Data Resource Center for Child & Adolescent Health .
Data presented here are based on 2024 National Survey of Children's Health (NSCH), which measures the share of children who recieve coordinated, ongoing, comprehensive care within a medical home. The presence of a medical home is determined from component indicators, including personal doctor or nurse, usual source for sick and well care, family-centered care, problems getting needed referrals, and effective care coordination when needed. To qualify as having a Medical Home children must meet the criteria for adequate care on the first three components: personal doctor or nurse, usual source for care, and family-centered care. Any children who needed referrals or care coordination must also meet criteria for those components in order to qualify as having a medical home. Missing values due to non-response or a “don’t know” response are not included in the denominator in calculations of prevalence estimates and weighted population counts. In the majority of cases, the proportion of missing values is less than 2%.

Definitions
Percent of Children with a Medical Home : Percentage of children ages 0-17 who received health care that meets criteria of having a medical home: child had a personal doctor/nurse; had a usual source for sick care; received family-centered care from all health care providers; had no problems getting needed referrals; and received effective care coordination when needed.
